Prostate Health: Screening and Prevention Strategies

Prostate Basics

The prostate is a walnut-sized gland that produces seminal fluid. As men age, the prostate often enlarges (benign prostatic hyperplasia), causing urinary symptoms. Prostate cancer is the most common cancer in men.

Screening Controversy

PSA testing (prostate-specific antigen) is the most common screening tool, but research shows it benefits some men while causing unnecessary anxiety and procedures in others.

Recommendations vary:

  • American Cancer Society: Discuss risks/benefits at 50 (or 40-45 if high-risk)
  • American Urological Association: Shared decision-making at 55-69
  • Most experts: Individualized approach based on risk factors

Risk Factors

  • Age (risk increases after 50)
  • Family history (dramatically increases risk)
  • African descent
  • Obesity and poor diet
  • Sedentary lifestyle

Prevention Strategies

Diet: Foods rich in lycopene (tomatoes), selenium (Brazil nuts), and healthy fats support prostate health.

Exercise: Regular physical activity reduces prostate cancer risk. Aim for 150 minutes weekly moderate activity.

Weight management: Obesity increases risk and poor outcomes.

Limit alcohol: Excessive alcohol may increase risk.

Avoid smoking: Increases aggressive prostate cancer risk.

Benign Prostate Enlargement

As men age, benign enlargement is common. Symptoms include:

  • Urinary frequency, especially at night
  • Weak stream
  • Difficulty starting urination
  • Incomplete emptying

Lifestyle modifications (reducing evening fluids, limiting caffeine and alcohol) often help. Medications or procedures are available if symptoms significantly impact quality of life.

When to See a Healthcare Provider

Discuss screening at your regular checkups. If experiencing urinary symptoms, seek evaluation to rule out infections or serious conditions.
